Job Description:
A company is looking for a Maternal Health Case Manager LPN.
Key Responsibilities
Make outbound/receive inbound calls to assess member's current
health status
Utilize motivational interviewing techniques to help members
identify and engage in positive behavior change
Document and track findings while coordinating care under the
direction of a Registered Nurse
Required Qualifications
Unrestricted licensure as a Licensed Practical Nurse in the state
of PA
2+ years of Case Management and/or Care Coordination experience
2+ years of Maternal and/or Pediatric clinical experience
Technical knowledge of Microsoft products including Excel and
Outlook
Must live in the state of Pennsylvania
