Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate care management activities for members with physical
needs.
Key Responsibilities
Evaluate member needs and barriers to care, recommending plans for
optimal outcomes
Develop and coordinate ongoing care plans, collaborating with
providers and community resources
Monitor member progress and update care plans as necessary to meet
goals
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
