Job Description:

We are seeking a skilled Registered Nurse to provide high-quality patient care in the comfort of their own homes in the Wyomissing, PA area. As a Registered Nurse with Omni Personal Care Services, you will have the opportunity to work 1:1 with patients, building trusted relationships and making a meaningful impact on their lives.Key Responsibilities:

• Provide clinical services within the scope of practice and in accordance with the plan of care/service plan.
• Conduct thorough clinical nursing assessments to ensure patient eligibility and medical necessity for services.
• Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements.
• Ensure plan of care frequency and duration meets the needs of the patient and initiates plan of care revisions as needed with physician approval.Requirements:
  • A minimum of one year experience as an RN preferred.
  • Current RN licensure in state of practice.
  • Current drivers license, vehicle insurance and access to a dependable vehicle or public transportation.
